How to Register:
1. Complete the registration form below.
2. Pay by mailing cash or a check to ESB Financial - P.O. Box 807, Emporia, KS 66801. If you are an ESB client, you can pay via bank transfer.

Details:
ENTRY FEE: $120 PER PERSON* ($480 per team)

This fee includes: 2 Mulligans per player, 1 miracle putt per team (automatic make from anywhere on the green on any hole), Green fees, Golf cart, Range Balls, Lunch Provided, Hole Games/Prizes, Player Pack that includes ESB Golf Tournament Specific Hat & Towel, etc.
